Physical Examination Requirements
According to the Illinois State Board of Education School Code:
PHYSICAL EXAMS AND IMMUNIZATIONS: Illinois law requires physical examinations for students entering kindergarten and sixth grade and for new enrollees from outside the State of Illinois. A copy of the health examination report, signed by a physician and dated within the past 12 months, must be on record with the District. Record of immunizations against measles, tetanus, diphtheria, poliomyelitis and pertussis (whooping cough) must also be present. All fifth grade students are required to have hepatitis B inoculations. If the health/immunization records are not in compliance with the Illinois School Code and the report is not received by October 15 of the school year, the student will be excluded from school until the report is in compliance. The exam must be reported on the Illinois Certificate of Child Health (or comparable) form, which is available from your school, your doctor, and from http://www.idph.state.il.us www.idph.state.il.us. Exams reported on Sports Physical forms are not acceptable for this purpose. For your convenience, you may download a copy of Illinois Health Exam Form [English] [Espanol].SPORTS PHYSICALS: Students in grades 6 through 8 who participate in interscholastic activities must have annual sports physicals on file with their school. Students will not be allowed to attend tryouts until an up-to-date physical is on file. Physicals may be reported on any form that lists body systems. A note stating that the exam was done is not sufficient. For your convenience, you may download the Hawthorn Middle School Interscholastic Physical form.
DENTAL EXAMS: Illinois law requires all students in kindergarten, second grade and sixth grade present proof of a completed dental examination before May 15 of the current school year. Note that students in grades 6, 7 and 8 whose records are not compliant with the dental exam requirement may be excluded from field trips and/or have their report card held. In addition, enrolling high schools may not grant admission until records are in compliance. Once your dentist completes the Illinois Proof of Dental Exam Form [English] [Espanol] , please return it to your school nurse. VISION EXAMS: Beginning with the 2008-09 school year, the state of Illinois requires mandatory vision examinations of all students entering school in kindergarten (or grade 1) or transferring from a school outside of Illinois. While the State is still working on the final process and paperwork, they have created a form the Hawthorn will use for the 2008-09 school year. Please have your optometrist or ophthalmologist complete the State of Illinois Eye Exam Report and return it to your school nurse. If you are unable to obtain the required vision examination, please complete the State's Eye Examination Waiver Form. Forms are also available from your school nurse.
