Illinois law requires physical exams for all students entering school. New enrollees from outside the State of Illinois have thirty days to present an exam that is less than a year old on a form comparable to the Illinois Certificate of Child Health Examination. A complete record of all immunizations is required, as well. Students who have transferred from within Illinois must present up to date health records upon registration in order to begin attending Hawthorn schools. More information about required physical, dental and vision exams is available on the Physical Exam health tab.
The nurse at each building reviews these records and notifies parents of any health records which are not in compliance with the Illinois School Code. Since Illinois immunization requirements may differ from other states or countries, we work closely with families to help them complete the requirements so that their child may attend school.
Please communicate any specific health issues directly with your nurse, so that we can all work together to support your child. More information is available on the Medications and Health Management tab.
Please contact the nurse in your child’s building if you need assistance in locating a health provider or see the listing in this website under Local Health Clinics.
