Rosalind Franklin University Health System (RFUHS) provides this mobile healthcare bus, which visits locations around the county. Services include free healthcare screenings and education. Immunizations are not currently available, though RFUHS hopes to offer that service at some point. No insurance required. Staff does not treat illness or injury. Stops include the YMCA (Vernon Hills, Waukegan, Lake Zurich) and Vernon Township.<br />

        <p>Illinois law requires physical exams for all students entering school. New enrollees from outside the State of Illinois have thirty days to present an exam that is less than a year old on a form comparable to the Illinois Certificate of Child Health Examination. A complete record of <a href="http://www.hawthorn73.org/index.php/healthservices/section/immunizations/">all immunizations</a> is required, as well. Students who have transferred from within Illinois must present up to date health records upon registration in order to begin attending Hawthorn schools. More information about required physical, dental and vision exams is available on the <a href="http://www.hawthorn73.org/index.php/healthservices/section/physicalexams/">Physical Exam</a> health tab.<br />
<br /><br />
The nurse at each building reviews these records and notifies parents of any health records which are not in compliance with the Illinois School Code. Since Illinois immunization requirements may differ from other states or countries, we work closely with families to help them complete the requirements so that their child may attend school.<br />

        <p>The Illinois State Board of Education School Code and Child Health Examination Code (77 Ill Adm. Code 665) requires specific immunizations for a child to attend public school. <br />
<br />
</p><div class="bodytitles">NEW REQUIREMENT:&nbsp; TETANUS/DIPHTHERIA/ACELLULAR PERTUSSIS (Tdap)</div><p>
Beginning with enrollment for fall 2012, any child entering 6th grade shall show proof of receiving one dose of Tdap (defined as tetanus, diphtheria, acellular pertussis) vaccine regardless of the interval since the last DTaP, DT or Td dose. Students entering grades 7 through 12 who have not already received Tdap are required to receive one Tdap dose regardless of the interval since the last DTaP, DT or Td dose. <br />
<br />
</p><div class="bodytitles">DIPTHERIA-PERTUSSIS-TETANUS (DPT)</div><p>
Any child entering a school program must show proof of having received at least three doses of DPT or DT, with the last dose having been given on or after the fourth birthday. There must be at least six weeks between the first and second doses and at least six months between the second and third doses.<br />
<br />
</p><div class="bodytitles">DIPTHERIA-TETANUS (DT)</div><p>
A tetanus booster is due ten years after the last booster.<br />
<br />
</p><div class="bodytitles">POLIO (IPV or OPV) </div><p>
Any child entering a school program must show proof of having received three doses of polio vaccine, with the last one being administered on or after the fourth birthday. The first two must be at least six weeks apart, and the third one must have been administered on or after the fourth birthday.<br />
<br />
</p><div class="bodytitles">MEASLES</div><p>
Children entering school at any grade level, K - 12 must show proof of having received two doses of live measles vaccine. The first must have been given at or after one year of age, usually as part of the MMR, and the second, no less than a month later. Proof of disease, verified by a doctor, is acceptable in place of immunization.<br />
<br />
</p><div class="bodytitles">RUBELLA (GERMAN MEASLES)</div><p>
Any child entering school at any grade level must show proof of having received one dose of rubella vaccine, usually as part of the MMR, given on or after the first birthday. Laboratory evidence (blood test) of antibody titer of 1:16 or greater is acceptable, in place of the vaccine. Reported history of disease is not acceptable proof of immunity.<br />
<br />
</p><div class="bodytitles">MUMPS</div><p>
Any child entering a school program at any grade level must show proof of having received one dose of mumps vaccine, usually as part of the MMR, given on or after the first birthday. Proof of disease, verified by doctor, is acceptable.<br />
<br />
</p><div class="bodytitles">HEPATITIS B</div><p>
Any child entering a school program below the kindergarten level OR entering fifth grade or later is required to be immunized for Hepatitis B. This is a series of three vaccines, given over a period of six months, with at least one month between the first and second doses. This series should be started in the school year prior to the requirement because of the length of time needed to complete the series.<br />
<br />
</p><div class="bodytitles">HIB (HAEMPHILUS INFLUENZA TYPE B)</div><p>
All children less than five years old entering a program below the kindergarten level must show proof of having received one dose of Hib vaccination, given on or after 15 months of age. The Hib is NOT required for children in grades K-12.<br />
<br />
</p><div class="bodytitles">VARICELLA (CHICKEN POX) VACCINE</div><p>
As of July 1, 2002, any child entering a preschool or kindergarten program is required to have the Varicella (chicken pox) vaccine or provide written documentation of having had the disease.<br />
<br />
</p><div class="bodytitles">TB SKIN TEST</div><p>
Mantoux Method. Recommended testing at entry into school.<br />

        <p>Medications, whether prescription or over-the-counter, require the completion of a <a href="http://download.hawthorn73.org/manager/download.php?file=720" target="_blank">Medication Authorization Form/Student Agreement</a>. A physician&#8217;s signature is required in many instances. It is also important that medication that comes to school is provided in original, labeled containers. Any medications held in the nurse&#8217;s office that are not picked up at the end of the year will be safely discarded.         <p>Illinois State law requires that schools screen students for vision problems in kindergarten, second, and eighth grades, as well as all new students, teacher or parent referrals, and students who receive special education services.<br />
<br /><br />
Hearing screening is required for students in kindergarten, 1st, 2nd, and 3rd grades, as well as for students receiving special education services, new students, and teacher or parent referrals.<br />
<br /><br />
The Nurses notify parents if results indicate that a student&#8217;s vision or hearing needs further evaluation. Financial assistance is available to families that do not have health insurance. Please ask the Nurse in your child&#8217;s building for information.

<div class="bodytitles">PHYSICAL EXAMS AND IMMUNIZATIONS </div><p>
Illinois law requires physical examinations for students entering kindergarten and sixth grade and for new enrollees from outside the State of Illinois. A copy of the health examination report, signed by a physician and dated within the past 12 months, must be on record with the District. Record of <a href="http://www.hawthorn73.org/index.php/healthservices/section/immunizations/" target="_blank">immunizations</a> against measles, tetanus, diphtheria, poliomyelitis and pertussis (whooping cough) must also be present. All fifth grade students are required to have hepatitis B inoculations. Beginning in late 2011, the Illinois Department of Public Health began requiring the specific Tdap vaccination (Tetanus/Diptheria/Acellular Pertussis vaccine) for students entering grades 6 through 12. If the health/immunization records are not in compliance with the Illinois School Code and the report is not received by October 15 of the school year, the student will be excluded from school until the report is in compliance. </p>

The exam must be reported on the <a href="http://download.hawthorn73.org/manager/download.php?file=1257">Illinois Certificate of Child Health Examination form</a>, which was updated in January 2011 to accommodate the new Tdap vaccine requirement (see above).&nbsp; Exams reported on IESA Sports Physical forms are not acceptable for this purpose. <i>Parents: Please complete the top of page one and page two, and sign page two of the form. Please keep a copy of the completed form for your records.</i><br />
<br />
</p><div class="bodytitles">SPORTS PHYSICALS</div><p>
Students interested in participating in interscholastic activities offered at our middle schools must have a current annual IESA sports physical on file with the school. Students will not be allowed to attend tryouts until an up-to-date physical is on file. A note stating that the exam was done is not sufficient. For your convenience, you may download the <a href="http://www.iesa.org/documents/general/IESA-PhysicalCard.pdf">IESA Sports Physical form.</a><br />
<br />
</p><div class="bodytitles">DENTAL EXAMS</div><p>
Illinois law requires all students in kindergarten, second grade and sixth grade present proof of a completed dental examination. Once your dentist completes the <a href="http://www.idph.state.il.us/HealthWellness/oralhlth/DentalExamProof10.pdf">Illinois Proof of Dental Exam Form</a>, please return it to your school nurse. The State of Illinois requires mandatory vision examinations of all students entering school in kindergarten (or grade 1) or transferring from a school outside of Illinois. Please have your optometrist or ophthalmologist complete the <a href="http://www.idph.state.il.us/HealthWellness/EyeExamReport.pdf">State of Illinois Eye Examination Report</a> and return it to your school nurse. If you are unable to obtain the required vision examination, please complete the State&#8217;s <a href="http://www.idph.state.il.us/HealthWellness/EyeExamWaiver.pdf">Eye Examination Waiver Form</a>.&nbsp; Forms are also available from your school nurse.         <div class="bodytitles">NURSE NEWS</div><p>
<i>January 2012</i>&nbsp; <b>NEW HEALTH FORM</b>&nbsp; To accomodate the new requirements for Tdap vaccine boosters (please read below), the state has developed a new <a href="http://download.hawthorn73.org/manager/download.php?file=1258">Certificate of Child Health Exam form</a>, which we are to begin using immediately. Make sure you use the one with January revision date (R-01-12) at the bottom. Parents should continue to fill in the top of both the first and second pages, answer the questions at the top of page 2, and sign after the answers.<br><br />

<i>November 1, 2011</i> <b>Free Mobile Community Care Connection Coach Back in Service:</b> The Rosalind Franklin University Health System has put the Community Care Connection mobile health bus back on the road. Formerly known as the Care Coach, the bus had stopped running last spring when Northwestern Lake Forest Hospital stopped providing the service. Services include free wellness screenings (blood pressure, cholesterol, blood sugar) and health education. They hope to be able to provide immunizations in the future. Discussions between the nurse, students, family members, or staff are private. All are welcome to talk openly about themselves, their health, or questions that they may have. The information is not shared with other people without permission. The only exception is if someone may be in danger. If a person prefers a more private place to talk, those arrangements will be made. Student medical records are confidential, as well. They are not released without parental consent. Written parental authorization is required for the school to obtain medical records from outside sources.<br />
<br />
</p><div class="bodytitles">ATTENDANCE</div><p>
Daily attendance is required by the School Code of Illinois. Student absences for any reason other than illness are strongly discouraged. If your child is going to be absent from school, parents must call the school by 9 am on each day of absence to report and explain the reason for the absence. The school phones have answering machines that can receive messages before and after school hours.<br />
<br />
</p><div class="bodytitles">PHYSICAL EDUCATION EXCLUSION</div><p>
If a student needs to be excused from P.E. or recess for a brief period of time (no more than three school days), parents may write a note, explaining the reason. For a longer exclusion, a physician&#8217;s note is needed. Notes should specify the reason for the exclusion and the duration. Both parent and physician notes are kept on file in the student&#8217;s health record in the Nurse&#8217;s office.<br />
<br />
</p><div class="bodytitles">MOVING</div><p>
If your family is moving outside of the school district, we would like to help make your transition as smooth as possible. When you are sure of your plans, please notify the school in writing. If you request it, the nurse will copy the most current health information for you to hand carry to the next school. This will simplify the medical portion of the registration process there.
