NEW REQUIREMENT: Tetanus/Diptheria/Acellular Pertussis (Tdap) Vaccination: Beginning with enrollment for fall 2012, any child entering 6th grade must now show proof of receiving one dose of Tdap (defined as tetanus, diphtheria, acellular pertussis) vaccine regardless of the interval since the last DTaP, DT or Td dose. Make sure you have this immunization listed on your 6th grade physical form, which is due before school starts in the fall. The Tdap vaccination is recommended for students entering 7th and 8th grade, but is not required. As further reminder, children entering 6th grade need to have a full physical (Illinois Child Health Exam form) and dental exam (Illinois Dental Exam form) and parents must also provide Proof of Residency. (Note that the Tdap requirement is a a revision of an earlier Illinois Department of Public Health ruling announced November 30, 2011 that had required all students in 6th through 12th grade to receive the Tdap vaccine.)
January 2012 NEW HEALTH FORM To accomodate the new requirements for Tdap vaccine boosters (please read below), the state has developed a new Certificate of Child Health Exam form, which we are to begin using immediately. Make sure you use the one with January revision date (R-01-12) at the bottom. Parents should continue to fill in the top of both the first and second pages, answer the questions at the top of page 2, and sign after the answers.

November 2011 Target Walk-In Clinic Open: The Vernon Hills Target Walk-in Clinic is now open. Services include treatment for minor illnesses and injuries, vaccinations, sports/camp physicals ($59), and more. Hours are Monday through Friday, 9 a.m. to 7:30 p.m. and Saturday/Sunday from 9 a.m. to 4 p.m. The clinic is closed each day from 1:30 to 2 for lunch. November 1, 2011 Free Mobile Community Care Connection Coach Back in Service: The Rosalind Franklin University Health System has put the Community Care Connection mobile health bus back on the road. Formerly known as the Care Coach, the bus had stopped running last spring when Northwestern Lake Forest Hospital stopped providing the service. Services include free wellness screenings (blood pressure, cholesterol, blood sugar) and health education. They hope to be able to provide immunizations in the future. October 15, 2011 Please remember that state law sets October 15 as the deadline for students to provide copies of all required health records. Students who are not in compliance must be excluded from school. Please remember to keep a copy of all records for yourself.

Note that every Hawthorn school is staffed with a school nurse. Parents who have specific medical conditions and needs to communicate with our nurses will find a variety of health management plans posted on this site.
May 18, 2011 Advocate Condell Immediate Care Sports Physicals: The Advocate Condell Immediate Care facility at 6 Phillip Road in Vernon Hills offers sports physicals for approximately $86. The site is open from 8 a.m. to 8 p.m. every day. No appointment is needed.
Children’s health is a key element in positive educational outcomes. The goal of Hawthorn District #73 Health Services and our School Nurses is to promote, protect, support, improve and sustain the health, emotional well-being and safety of all students and staff for optimal health, growth, learning, and quality of life. This is accomplished in collaboration with the students, families, staff, administrators, and community resources.
Parents are asked to provide important health and medical information about their student, which is updated each year during the online registration process. Please complete this information as accurately as possible, as this information will accompany your child to the hospital if emergency treatment is needed. It also enables the Nurse to understand your child’s health needs at school. If there are changes during the school year, please be sure to notify the Nurse. This health information is maintained by the Nurse in each building and is kept confidential. Please feel free to contact the Nurse at your child’s school regarding any concerns about your child’s health or medical needs.

Discussions between the nurse, students, family members, or staff are private. All are welcome to talk openly about themselves, their health, or questions that they may have. The information is not shared with other people without permission. The only exception is if someone may be in danger. If a person prefers a more private place to talk, those arrangements will be made. Student medical records are confidential, as well. They are not released without parental consent. Written parental authorization is required for the school to obtain medical records from outside sources.
Daily attendance is required by the School Code of Illinois. Student absences for any reason other than illness are strongly discouraged. If your child is going to be absent from school, parents must call the school by 9 am on each day of absence to report and explain the reason for the absence. The school phones have answering machines that can receive messages before and after school hours.
If a student needs to be excused from P.E. or recess for a brief period of time (no more than three school days), parents may write a note, explaining the reason. For a longer exclusion, a physician’s note is needed. Notes should specify the reason for the exclusion and the duration. Both parent and physician notes are kept on file in the student’s health record in the Nurse’s office.
If your family is moving outside of the school district, we would like to help make your transition as smooth as possible. When you are sure of your plans, please notify the school in writing. If you request it, the nurse will copy the most current health information for you to hand carry to the next school. This will simplify the medical portion of the registration process there.
