Medication Requirements at School
All prescription and over-the-counter medications given at school require a doctor’s order. Families must complete the Medication Authorization Form/Student Agreement and have it signed or stamped by the student’s doctor. Medication must be brought to school by an adult in its original, labeled packaging. Medications not picked up by the end of the school year will be safely discarded.
For students with specific health needs, please download and complete the applicable plan below and return it to your school nurse.
