How to Submit Medical Forms

You can submit your child's medical forms in one of the following ways:

  1. Upload: Submit the forms when registering in PowerSchool.
  2. Email: Send the forms to medicalrecords@hawthorn73.org.
  3. In-Person: Turn in the forms at your child's school front office or the District Office.

Physical Exams and Health Forms 

Illinois law requires physical exams for students entering kindergarten, sixth grade, and for new enrollees from outside Illinois. A health examination report, signed by a physician and dated within the past 12 months, must be on file with the District. Immunization records for measles, mumps, rubella, diphtheria, tetanus, pertussis, polio, and varicella are required. Sixth graders must also have completed the hepatitis B series, a Tdap booster, and one meningococcal conjugate vaccine (MCV) given on or after their 11th birthday.

If health records are not compliant with Illinois School Code and submitted by October 15, the student will be excluded from school until compliance is met. The exam must be documented on the Illinois Certificate of Child Health Examination form. IESA sports physical forms are not acceptable.

Annual Vision and Hearing Screenings

Hearing and vision screenings are provided annually to District 73 students at state-mandated grade levels, as required by the Illinois Department of Public Health. These screenings, conducted by state-certified nurses, are not a substitute for a full eye exam by an eye doctor or a hearing exam by an audiologist.

Vision Screenings:

  • Students in Grades: Kindergarten, 2nd, and 8th
  • Students receiving Special Education Services
  • Students transferring to Hawthorn District 73 from out of state or country
  • Any student suspected of having a vision concern by a parent, guardian, or teacher

Hearing Screenings:

  • Students in Grades: Kindergarten, 2nd, and 8th
  • Students receiving Special Education Services
  • Students transferring to Hawthorn District 73 from out of state or country
  • Any student suspected of having a hearing concern by a parent, guardian, or teacher
